1. That the Court shall hold a hearing in the above-captioned matter on July 16, 2014 at 2:00 p.m. in Courtroom A at the Bristol Superior Court, located at 441 County Street, New Bedford. Anyone claiming to have an interest in the Corporation may appear at the hearing and request to be heard.

2. At that hearing, the Court shall address whether to allow the request of the Receiver to dissolve the Corporation and to declare that all shares in the Corporation issued after the effective date of St.1947, c. 559, § 1 (approved on June 20, 1947) (the “1947 Act”) be declared null and void ab initio on the basis that non-profit corporations were without authority to issue shares after the effective date of the 1947 Act. The Court shall also consider the Receiver’s request to pay holders of shares issued after the effective date of the 1947 Act fifty dollars per share (or some other amount deemed appropriate by the Court) in exchange for the holder’s release of all claims and rights relating to or deriving from such shares and will address whether to issue other appropriate orders in connection with the dissolution of the Corporation and the distribution of its assets.

3. Anyone claiming an interest in the Corporation who objects to the Polish Home being dissolved, objects to the post-1947 Act shares being declared null and void, or objects to the payment of monies to the post-1947 shares shall, at least seven days prior to the hearing, file with the Civil Clerk’s Office a written objection. The written objection shall be filed at:

The written objection shall identify the matter by the names of the parties and the civil action number stated above and shall state all reasons supporting such objection and attach copies of any and all supporting documentation.

4. Contemporaneously with the filing, the objector shall also mail or otherwise deliver a full copy of the filing to William T. Harrington, Esq., 171 Milk Street, 2nd Floor, Boston, MA 02109.

5. All shareholders or purported shareholders in the Corporation and anyone else claiming an interest in the Polish Home are requested to fill out and sign, under the pains and penalties of perjury, the appropriate attached form, with Member Clubs filling out Form A, individuals holding shares initially issued after 1947 filling out Form B, and individuals holding shares originally issued after 1947, filling out Form C. The appropriate form, along with copies of supporting documents, shall be delivered, at least seven days prior to the hearing, to:

In the alternative, the form and supporting documents may be emailed in PDF format to [email protected] The form and supporting documents should be attached to the email in PDF format. The head of the email shall state "Polish Home Shareholder Information." Such forms and documents shall not be filed with the Court, unless otherwise ordered by the Court.
